Things to remember before completing this form:

  • To be eligible for the Rehab Program, you must be a qualified health practitioner.
  • You get a deeper discount for signing up for more courses at once. Go here to find out more information. Students wishing to complete all four discounted Instructor Streams will have 18 months to do so.

  7. Have you taken one or more of the STOTT PILATES rehabilitation courses listed on this application (below) at Pacific NW Pilates before?
  8. New applicants to Pacific NW Pilates:You must answer ALL questions in the application below. If you have never taken a STOTT PILATES certification course anywhere, you must submit a $50 processing fee with your application before we will begin processing it. If you have already completed at least one official STOTT PILATES course at another location, you do not need to include a letter of reference or an application fee.

    Returning students to Pacific NW Pilates:You must submit an application that provides your current contact information only. You do not need to pay the application fee.

    All applicants:You must provide official documentation of your qualifications as a health professional. (see below) Space will ONLY be reserved in a course upon receipt of your tuition deposit (following acceptance to the program if you are a new student). All Rehab courses require a tuition deposit of $250.

    If you are a NEW certification course student at Pacific NW Pilates, you must fill out the remainder of this form AND fill out the anatomy quiz.

    You will not be registered for a course until we've received your tuition deposit.

    You will not be admitted to a course until your full tuition payment has been received.
  17. Pacific NW Pilates Course Application Policies
  18. 1. Application fees for new students are non-refundable.

    2. All students must read the Policies and Procedures document before submitting their application. This document is updated annually and must be read prior to submitting each application for each course that the student applies for. By submitting your application you are agreeing to all terms contained therein.

    3. A tuition deposit must be paid to reserve your spot in a course. If you are signing up for courses individually (not as a part of a stream), each Rehab course requires a $250 deposit. If you are signing up for courses as a stream, you may be eligible for a payment plan. In this case, 20% of the total cost is required to hold your place in the courses.

    4. Students must either pay fully or pay the initial deposit and set up a payment plan two weeks (14 days) prior to the course start date to receive stream discounts on the courses for which they are applying.



    5. Students that decide to take courses individually instead of as part of a stream, will not be eligible for the discounted stream prices or "add on" prices at a later date. For example, if a student decides to take RMR1 as an individual course, he or she may not go back at a later date and add on the RMR2 at the discounted rate; he or she will be charged the full course and retail prices on both courses.

    6. Students must withdraw from a course 14 days prior to the course start date in order to receive a full refund on materials and tuition. Please note, returned materials must be in re-sellable condition. Late cancellations, schedule changes and/or absence from a course will result in forfeiture of the tuition deposit or require make-up time at an additional fee. In addition, the remainder of the course tuition and materials fees are non-refundable but may be applied to another course or workshop at Pacific NW Pilates within a 12-month period.

    7. Pacific NW Pilates reserves the right to cancel a course or workshop up to one week (7 days) before the start date. We highly recommend that students traveling to attend a course or workshop discuss or confirm scheduling with PNWP staff before making travel arrangements.

    8. In the event of a course cancellation, students are eligible for a full refund on tuition and materials, or students may be applied to a future course or workshop at Pacific NW Pilates. Please note, returned materials must be in re-sellable condition.

    9. Tax for retail items and course supplies will be applied for courses held in states other than Oregon.
